- NHS Prescribing Criteria for Mounjaro (Tirzepatide)To be prescribed Mounjaro on the NHS at your GP surgery, you must meet all of the following requirements:
- BMI of 40 or above (BMI threshold reduced by 2.5 kg/m² for people from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds).
- Have at least four of the following weight-related health conditions recorded in your medical history:
- Atherosclerotic cardiovascular disease (ASCVD)
- Hypertension (high blood pressure requiring medication)
- Dyslipidaemia (high cholesterol)
- Obstructive sleep apnoea (confirmed by a sleep clinic)
- Type 2 diabetes mellitus
(You can check your BMI using the NHS BMI calculator, or your height and weight will be checked at your appointment. Accurate height and weight information is required—please provide this when you book and attend.)
Referral to a Specialist (Tier 3) Weight Management Service
If you do not meet the above criteria for Mounjaro in primary care, you may still be eligible for referral to a Tier 3 weight management service. These services offer specialist support and advice.
Standard referral criteria:
- Age 18 or over.
- BMI of 40 or above with four or more qualifying comorbidities (as above).
Fast-track referral criteria:
- BMI of 35 or above plus at least one of the following:
- Intracranial hypertension (sight-threatening) requiring frequent lumbar punctures and/or with visual compromise
- Awaiting transplant surgery and requiring BMI reduction to 30–35
- Obesity-dependent malignancy (e.g. endometrial cancer where high BMI is the primary barrier to surgery)
- BMI of 50 or above seeking and potentially eligible for metabolic surgery.
